ALACHUA COUNTY, FL - The Alachua County Commission is seeking applications from governmental entities, or 501(c)(3) agencies (with a physical address in Alachua County) to deliver health care services for County residents during the FY2016/2017 grant year. Included categories of services are medical services, oral health, and mental and behavioral health.

Deadline for application submission is August 24, 2016, by 5 p.m. (no late applications will be accepted). Applications must be submitted on a USB Flash drive and must include one original hardcopy of each program application. Applications must be delivered to the Alachua County Purchasing Division.
