The site is in the R-1A (single-family residential) zoning district and is located at 6517 N.W. 39th Avenue on tax parcel numbers 06184-002-000, 06193-000-000, 06194-004-000, 06194-005-000 and 06194-006-000. The request, if approved, will allow Institutional uses (medical, community services, government buildings, religious structures) on this property. This application is associated with ZOM-04-17. ZOM-04-17 (Planned Development District) – This is a request by CHW, Inc., agent, for Pradeep and Marta Raval, owners, to rezone from R-1A (single-family residential) district to a non-residential PD (Planned Development) district. The site is 20.42 acres and has a future land use designation of Medium Density Residential (4-8 dwelling units/acre). The site is located on tax parcel numbers 06184-002-000, 06193-000-000, 06194-004-000, 06194-005-000 and 06194-006-000. If approved, the PD would permit a 223 bed skilled nursing facility on the site. This request is being heard in conjunction with CPA-06-17, a large scale Comprehensive Plan amendment.
