The Alachua County Board of County Commissioners is seeking proposals from governmental and non-profit 501(c)(3) agencies with a physical address in Alachua County for delivery of programs to aid in the reduction of health disparities and lack of access to care in the categories of medical services, oral health, and mental/behavioral health. Non-profit agencies must have a current IRS 501(c)(3) designation.
All CHOICES applications must be completed electronically via Zoom Grants beginning 12:00 PM May 10, 2022 and submitted no later than 5:00 PM May 31, 2022.

No late proposals will be accepted. Proposals that do not meet the grant criteria will be rejected. Application rules and instructions, including the complete Request for Applications (RFA) document, are available on the site and interested vendors are encouraged to read all related documents prior to beginning an application.
CHOICES is a reimbursement grant, requiring grantees to expend their own funds and request reimbursement. The grant period is from October 1, 2022, through September 30, 2025, subject to annual appropriation by the Alachua County Commission.
An optional information session will be held for interested agencies. The session will cover the application process, using the application site, and tips for a successful proposal.
