The Alachua County Commission is seeking proposals from governmental and non-profit 501(c)(3) agencies with a physical address in Alachua County for delivery of programs to aid in the reduction of health disparities and lack of access to care in the categories of medical services, oral health, and mental/behavioral health. Non-profit agencies must have a current IRS 501(c)(3) designation.
All FY 19-20 Community Health Offering Innovative Care and Educational Services (CHOICES) applications must be completed electronically via Zoom. Grants will be accepted beginning at noon on June 12, 2019, and must be submitted no later than 11:59 p.m. on July 10, 2019.

No late proposals will be accepted. Proposals that do not meet the grant criteria will be rejected. Application rules and instructions, including the complete Request for Applications (RFA) document, are available on the site. Interested vendors are encouraged to read all related documents prior to beginning an application.
The CHOICES grant is a reimbursement grant, requiring grantees to expend their own funds and request reimbursement. The grant period is from October 1, 2019, through September 30, 2022, subject to annual appropriation by the Alachua County Commission.
An optional information session will be held for interested agencies on Tuesday, June 25, 2019, at 10 a.m. at the Alachua County Community Support Services’ Conference Room A (218 S.E. 24th Street, Gainesville).
