​​​​​​​​​​​​The Clerk's office is pleased to make available the Supreme Court approved forms for your use. In County Court, many small claims and county court forms are available. However, because of the complexity of most Circuit Court Cases, only Family Law forms have been approved for distribution for that court.

These forms only apply to sealing or expunging court records under sections 943.0585 and 943.059, Florida Statutes. There are other forms of sealing that may apply to you. Please consult with an attorney if you believe the facts in your case do not match the requirements of these statutory sections.​

EMAIL SERVICE FOR COURT DOCUMENTS REQUIRED
Fla. R. Gen. Prac. And Jud. Admin 2.516 (amended by SC21-990) requires that all documents be served by email, but a party may request to be excused if they do not have an email address or regular access to the internet.​
